Medical and Disability Management

The initial step in successful Disability Management is Medical Management. The process begins at the time of referral. One of our highly skilled Case Managers here at M. Salerno & Associates, Inc. initiates contact with the patient and conducts an initial meeting at his/her home, or at the hospital/facility in which he/she resides. The patient’s current medical condition is assessed, as is the appropriateness and quality of the current medical care being provided. The assigned Case Manager will also meet with family members, and the entire interdisciplinary treatment team.

Following the initial assessment of the case, the Case Manager makes recommendations for continued medical care and promptly communicates this information to the payer. Some case management recommendations might include:

Performing a home assessment to determine the patient’s needs which will promote independence

Arranging a second opinion, if necessary
In the premature baby cases, we work closely with the parents and nursing staff to facilitate discharge as soon as medically possible. Ongoing communication with the carrier is vital to keeping them informed of the infant’s progress and prognosis
Coordinating a transfer to a more medically appropriate facility. This will save the carrier money by improving the quality of care while minimizing costs of inappropriate or inadequate treatment
Arranging discharge from facility care to the home setting and coordinating appropriate home care services and equipment
Assisting a patient with continued medical care, if needed
Providing information to the patient with regard to outside resources associated with their particular medical problem and/or disability
Negotiating lower costs for services/equipment
