Every file is managed using a pro-active methodology that begins immediately upon referral to our office. Fragmentation of care occurs when medical treatment is sought without the benefit of early onsite assessment by an experienced nurse Case Manager. Early intervention is the most cost-effective and efficient way to move a case toward successful resolution.
